Note: If you have already submitted any of these documents for a prior BVDC event in 2025, you do not need to re-submit (however, remember that everyone in your group must sign the waivers). CLICK HERE to confirm which documents we already have for you.

• For each equine:

Copy of current negative Coggins test (within one year) and Flu/Rhino documentation (within 6 months) per ADS rule GR-6.3

• Signed Club Release - Sign either BVDC or SJCC waiver.

For BVDC MEMBERS: Everyone in your group must sign this (you can use one form for all signatures). CLICK HERE to download the form so you can print it.

For SJCC MEMBERS OR IF YOU ARE NOT A MEMBER OF EITHER CLUB: Everyone in your group must sign this (you can use one form for all signatures). CLICK HERE to download the form so you can print it.
